It is with mixed emotions and sadness that I am announcing my retirement from my practice at Clinics of North Texas, effective May 29, 2026. It has been an honor and a great pleasure getting to know you and your families and providing for your health care needs over the years.
If you have any appointments scheduled with me at the clinic after May 29, 2026, those appointments have been cancelled. It is possible that due to the timing of this notice, you may have received a reminder for those appointments; however, regardless of that reminder, any appointments have been cancelled.
You may contact the Wichita County Medical Society (940-723-7072) for the names of physicians who are accepting new patients.
Your medical record will be maintained at the Clinics of North Texas. To transfer care to a physician outside the Clinics of North Texas you may obtain a copy of your medical record by signing a written authorization form. An authorization form may be obtained from our Medical Records department which is located in the basement at 501 Midwestern Parkway E or located on our website under the patients and visitors tab, titled Authorization for Release of Information Form.
I have greatly valued our relationship and the opportunity to care for your family. I thank you for your loyalty and friendship over the years and wish you all the best for a happy and healthy future for you and your family.
